Across more than ten procedures observed in the field, at least half of the instruments in a surgical set were never used. Every one of them was still washed, inspected, wrapped, sterilised, transported and counted.

The reason sets do not get trimmed is not indifference. It is that nobody has the evidence. Usage is not recorded, because recording it by hand during a case is not realistic, and recording it on video creates a data-protection problem larger than the one it solves.
